Pre-registration Procedure

Pre-registration with the practice

Please call the reception on 020 8422 1813 to check if you are in our practice’s catchment area or not.

If you are in practice’s catchment area, the receptionist will offer you an appointment for the registration.

On the day of your appointment, please bring the essential documents (see attached):

You can print off a registration form, fill it out and bring it in with you on your first visit to the practice.

* Please make sure you fill the registration form in “BLOCK CAPITALS”.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.  When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
